Some details that you provide in this questionnaire will be included in White Coat Ceremony commemorative information (see below). The additional questions (optional) help us select featured students for stories that will be used to promote the Class of 2030 and the college's DVM program. To ensure accuracy, please verify all of the information that you provide in your responses. Before you send in your questionnaire, take a few minutes to go through this check list:

• Did you provide your full name and correct contact information? If possible, please provide your current email address.
• Did you include "N/A (not applicable)" in the response windows for questions that did not relate to your experiences?
