Introduction
As a staff member of a Project AMPLIFI hospital or Long Term Care site who has implemented the PointClickCare solution and has treated residents from or discharged patients at either end of the Long-Term Care to Hospital care transition, you are invited to participate in the Project AMPLIFI research survey to evaluate the effectiveness and utilization of the PointClickCare data integration solution.
Patients are regularly transferred between hospitals and long-term care homes to receive the most appropriate care as part of their journey through the health system. The Project AMPLIFI study aims to review and assess implementation outcomes (utilization statistics, clinician feedback, and patient transfer data).
Answer all questions to the best of your ability. This survey should take you approximately 10 minutes to complete.
Survey data will remain anonymous. For those respondents agreeing to participate in the optional interview, all data collected will be de-identified from the contact information provided. All data will be published in aggregate form only.

The survey has been reviewed by Veritas Independent Review Board (IRB).
