Sexually transmitted and blood-borne infections (STBBI) test results request form
The sexual health services test result policy at the Southeast Public Health is “no news is good news”. We will only contact you if your test result is abnormal or requires follow-up. Please do not contact the clinic for your results for at least two weeks after your test was taken.
If you need a copy of your test result, please fill out this form and a nurse will follow up by your preferred option (email or phone).
Please note that this form is only for STBBI test results provided by Southeast Public Health, or the legacy health units:
- Hastings Prince Edward Public Health
- Kingston, Frontenac, Lennox & Addington Public Health
- Leeds, Grenville & Lanark District Health Unit
