To submit a freedom of information request, please complete the form below only for services provided by Southeast Public Health, or the legacy health units:
  • Hastings Prince Edward Public Health
  • Kingston, Frontenac and Lennox & Addington Public Health
  • Leeds, Grenville & Lanark District Health Unit
Access to Information requests for other organizations should be directed to their respective offices.
