Purpose of This Survey
This survey is designed to facilitate the evaluation of courses completed at other institutions for prerequisite equivalency within our program. Please be advised that submissions that are incomplete or contain incorrect information will not be reviewed.

Before proceeding, ensure that you meet our program-specific residency requirements.


Eligible Course Categories for Assessment
This form is intended solely for the evaluation of courses in the following subject areas:
Speech-Language Pathology Occupational Therapy Physical Therapy
Anatomy ✔️ ✔️
Anat/Neuroanatomy ✔️ ✔️
Statistical Methods ✔️ ✔️ ✔️
Phonetics ✔️
Linguistics ✔️
Human Physiology ✔️
Speech-language pathology applicants (ONLY) may fulfill the prerequisite requirement for Anatomy with either an Anatomy OR a Neuroanatomy course—only one is required, not both. However, because these categories differ in content, each category must be evaluated independently.

For courses in Social Sciences, Humanities, Indigenous Studies, or Psychology, please contact us directly at rs.admissions@usask.ca with the course number and title.

Before You Begin: Required Materials
To ensure your submission is processed efficiently, please have the following ready:
  • A complete course outline, including a weekly breakdown of topics. Each syllabus must clearly display the university name and course title.
  • Please read all requirements carefully. Incorrect or incomplete submissions may prevent your assessment from being processed.
