1. Welcome

Hello!

We are Shawn Pegg (Community Living Ontario) and Jess Whitley (University of Ottawa). We are working together to learn about the school experiences of students with disabilities, neurodiversities, exceptionalities, and other support needs in Ontario.

We are especially interested in experiences of exclusion, restraint, and seclusion at school. What we learn will help schools better support children and families.

As a parent, caregiver, or guardian of a child in an Ontario school, your experiences and perspectives are important!

How long does the survey take?
The survey takes about 10–25 minutes. If you would like to tell us more, you can volunteer for an interview by clicking here (option at the end of the survey as well). If you have more than one child who fits the criteria, please complete a separate survey.

What are the risks and benefits?
Some questions may bring up difficult feelings or memories. You can skip any question or stop the survey any time. We have provided a list of mental health resources, and you can contact Shawn Pegg (shawn@communitylivingontario.ca) for information about family support and advocacy. Many people find it helpful to share their experiences. We will share what we learn with governments and school boards and hope it helps improve supports for children and families across Ontario.

Can I complete the survey over several days?
No. The survey must be completed in one sitting and cannot be saved to finish later.

Will my information be shared?
No. Your survey responses will only be seen by the University of Ottawa research team and will not be shared with governments, school boards, or anyone else. Because the survey is anonymous, we can’t remove your responses after you submit them. Survey responses will be stored securely on SurveyMonkey until August 2026. They will then be downloaded to a password-protected uOttawa computer. A summary of the survey data will also be stored securely on Microsoft OneDrive for 10 years.

Will I be paid for my time?
As a thank you, you can enter a draw to win one of thirty $15, five $25, or one $100 Amazon gift cards (total value: $675). You can enter the draw even if you do not complete the survey. If you enter the draw, your email address will only be used to contact you if you win. It won’t be linked to your survey responses and will be deleted after the prizes have been awarded. To enter the draw, please click here.

What if I have questions or need help filling out the survey?
Contact Jess at inclus@uottawa.ca, or Shawn at shawn@communitylivingontario.ca. A research assistant can help you complete the survey.

If you have concerns about the research, contact the University of Ottawa Research Ethics Board at ethics@uottawa.ca or 613-562-5387.
