Introduction
Thank you for taking the time to share your experiences with Northern Health (NH). This survey will help us understand how we can improve Inclusion, Diversity, Equity, and Accessibility (IDEA) across our region. Your responses will directly inform our three-year IDEA strategy and annual action plans.

Some topics may involve sensitive issues such as discrimination, barriers to care, or exclusion. These questions are intended to help us understand and address challenges within our communities.

Your participation is voluntary, and you may skip any question you do not wish to answer. If at any point you feel uncomfortable, you can stop the survey. All responses will remain confidential and will only be used to inform strategies that promote equity and inclusion.

  • Who is collecting the information? NH's Quality and Transformation team.
  • For what purpose? To gather feedback that will shape NH's IDEA strategy.
  • Who are we collecting information from? NH staff and medical staff, patients, family members, and community members.
  • Is the survey voluntary? Yes, participation is completely voluntary.
  • How long will the survey take? Approximately 8-10 minutes.
  • Survey deadline: July 22, 2026 at 5:00 pm DST.

NH collects your response to this survey under section 26(e) of British Columbia's Freedom of Information and Protection of Privacy Act (FIPPA) for the purpose of planning and evaluating NH’s IDEA work. NH does not intend to identify you in this survey. If you have unique qualities based on your demographic responses, there is a small chance you may be indirectly identified. Please do not include your name, the names of other people, or details about dates and times from your answer to the open-text question of this survey. Any personal details included in open-text responses will be reviewed and removed before reporting.
