Volume 4: Sustaining Communities of Care

Strengthening the people, organizations, and systems that sustain care, well-being, and health equity in our communities.
Care happens everywhere. In our homes, communities, cultural spaces, health organizations, and through the work of caregivers, community workers, advocates, and grassroots leaders.

Yet many of the people and organizations sustaining care are doing so with limited resources, fragmented systems, and little influence over the decisions that shape how care is supported.

Reimagine Health 2027: Sustaining Communities of Care brings together people working across health, community care, policy, advocacy, and grassroots initiatives to explore a central question:

What would it take to build and sustain communities of care that are equitable for both the people receiving care and the people providing it?

Through Equity-Centred Community Design, systems thinking, shared learning, and collaborative problem-solving, participants will explore challenges, build relationships, and develop and test ideas for change.

You don't need to have a fully developed idea or work within the formal health-care system to apply. We welcome people bringing lived experience, community knowledge, professional experience, and a curiosity about creating change.
