Complete this Vendor Request Form to see if you’re eligible to join us for a monthly innovation meeting. If so, you will be invited to a 55-minute meeting. Meetings can be held virtually, on-site or at your location within the Durham Region.

Our interdisciplinary team members will review and provide feedback on your product. Those who meet the needs of our clients will be considered by our Interprofessional Practice and Outcomes Committee.
