Please print off a copy of this form for your records.

Researchers:
Kathrina Mazurik, PhD Student, Department of Psychology; Research Intern at Cadence, kathrina.mazurik@usask.ca
Rachelle Perron, Visiting Student, Department of Psychology; Co-founder of Cadence, rachelle.perron@cadencecares.ca
Supervisor: Michel Desjardins, Associate Professor, Department of Psychology, University of Saskatchewan, michel.desjardins@usask.ca

Purpose and Objective of the Research:
The purpose of the research is to better understand administrative work preceding and following death. This information will be used both for academic purposes (developing a general understanding of this task) and for applied purposes (developing and refining a service that would support professionals and executors with this process)

Procedures:
Participants will take part in a single 5 to 10 minute online survey. The questions focus on practical end of life planning and (if relevant) experiences of executor duties. Demographic information will be collected to describe our sample, to determine representativeness, and to conduct analyses to examine for differential response patterns across respondents from different demographic strata.

Funded by:
This research is being funded in part by Mitacs (an organization that creates bridges between universities and industry) and in part by Cadence (a Canadian business that provides services to support executors with executor duties).

Potential Benefits:
Benefits of the study include informing the state of knowledge on the topic and informing the development of digital services that could support bereaved individuals and the funeral directors who support them. These benefits are not guaranteed.

Risks:
There are no known risks associated with this study.

Compensation:
Should you wish, you may choose to enter a draw for the chance to win 1 of 6 $50 VISA gift cards. When you finish the survey, you will be automatically redirected to a page that will collect your email address for the gift card draw.  The information collected on this page will be stored separately from your survey responses.

Confidentiality:
Your survey responses will be completely anonymous and any identifying information you provide for the purposes of the draw will be stored separately. The data collected for this study may be published in an academic journal or white paper and/or presented at a professional conference.

Storage of Data:
This survey is hosted by Survey Monkey. Your data will be stored in facilities hosted in Canada. Please see the following for more information on the Survey Monkey Privacy Policy. The anonymous electronic data will be stored on a password-protected computer during analysis, but moved to OneDrive for long term storage. OneDrive’s servers and no data will be transmitted through, or stored on, any servers outside Canada. All of the data from the survey will be securely stored until five years after the study is published.  It will then be destroyed beyond recovery through the use of Eraser, a free file eraser program that overwrites the deleted files on personal computers/laptops and portable media.
Your contact information (for the purposes of the draw) will be saved to a password-protected and encrypted Master List on the University of Saskatchewan OneDrive cloud storage, separate from the data. This information will only be accessible to the researcher and her supervisor. Once the draw is complete and this information is no longer required, the Master List will be destroyed.

Right to withdraw:
You may withdraw from the study for any reason, at any time prior to submitting the data. If you wish to withdraw from the study simply close your web-browser without submitting your data. However, due to the anonymous nature of the database, you will not be able to withdraw after you have submitted your data because of the inability to identify the data of any specific individual.

Questions:
If you have any questions concerning the study, please feel free to ask at any point by contacting the researchers by email.  You are also free to contact the researchers at the numbers provided above if you have questions at a later time. This research project has been approved on ethical grounds by the University of Saskatchewan Research Ethics Board (Beh-REB 2841).
