This Privacy Notice is provided to inform you about how your personal data will be collected, used, disclosed and protected by FCM. Please read the following information carefully.
  • Purpose of Data Collection - The data collected through this survey will be used by FCM to verify eligibility of applicants and appointees, inform the appointment of non-board members, and understand the diversity and representation of board directors and non-board members. Your participation in the demographics section is voluntary, and you may choose not to answer any question with which you do not feel comfortable.
  • Use of Your Data - The personal data you provide in this survey will be used by FCM solely for the purposes stated above. In certain limited circumstances, we may disclose personal data with other third parties, including (a) to comply with our legal obligations, to protect the rights and property of FCM and the public, to cooperate with law enforcement investigations, and to detect and respond to suspected illegal activity and threats to the health or safety of any person or of our systems or services; and/or (b) with your consent and at your direction. We may also disclose aggregated or de-identified information, which cannot reasonably be used to identify you.
  • Data Storage and Security - Your responses will be stored securely and will be accessible only to authorized personnel. All data will be retained only for as long as is reasonably necessary for the purposes set out above or to comply with our legal obligations, after which point it will be securely disposed of or archived in accordance with applicable law.
